Job Title: Case Manager
Full-Time, Non-Exempt

Purpose of the Role
The Case Manager supports program participants in setting, pursuing, and reaching their goals toward financial independence and self-sufficiency. This position may require home visits, a flexible work schedule, and availability during evenings or weekends to meet participant needs.
Key Responsibilities
• Conduct eligibility screenings for prospective participants and provide detailed program information.
• Collect and verify required documentation, including income and authorization forms; review with supervisor.
• Complete assessments such as HMIS Intake, VI-SPDAT, and Housing Retention Assessment to identify participant strengths, needs, and risks.
• Interview participants and gather relevant information to develop individualized service plans.
• Analyze assessment data to create comprehensive, participant-centered plans.
• Understand and evaluate participant health and social risk factors using appropriate risk stratification.
• Provide guidance on health and social service systems.
• Establish goals aligned with participant needs, available resources, and required level of care.
• Document all interactions and updates in the case management system accurately.
• Communicate care plans to internal staff as needed.
• Complete case planning tasks and updates within established timelines.
• Conduct regular reassessments of participant progress, adjusting plans as needed.
• Connect participants to community resources and organizational services.
• Proactively recommend adjustments to care plans when progress stalls or regresses.
• Maintain accurate, timely documentation in case files.
• Communicate eligibility changes for services or financial assistance.
• Uphold confidentiality and compliance with legal reporting requirements.
• Facilitate case closure based on established criteria, including successful program completion or voluntary exit.
• Participate in case conferences and coordination meetings.
• Stay informed about best practices in housing and social services.
• Maintain connections with internal and external resource partners.
• Manage a caseload of 20–50 households, depending on service type.
• Provide education on budgeting, financial management, tax credits, and credit repair.
• Assist participants in accessing public and private resources to support stabilization.
• Ensure minor children are enrolled in school and connected to needed services.
• Report challenging participant issues to supervisors with recommendations for service adjustments.
• Assist in preparing progress reports for internal and external audiences.
• Attend required and recommended trainings.
• Follow procedures for opening new service files.
• Maintain accurate participant data in the HMIS system; complete entries within 24 hours of service delivery.
Required Skills
• Knowledge of social service systems and self-help strategies.
• Ability to independently counsel individuals experiencing mental health, substance use, or homelessness challenges.
• Understanding of public benefits and financial resources.
• Strong motivational interviewing skills.
• Crisis intervention, conflict resolution, and trauma-informed care experience.
• Bachelor’s degree in social services or related field, OR equivalent combination of education and experience supporting individuals facing crisis or trauma.
• Experience working with low-income and/or homeless populations.
• Proficiency with Microsoft Office (Outlook, Word, Excel).
• Ability to work flexible hours, including evenings and weekends.
• Experience working with diverse populations and partner agencies.
• Reliable transportation and a driving record that meets insurance requirements.
• Ability to pass a criminal background check.
Preferred Qualifications
• Master’s degree in social services, psychology, or related field.
• Experience with low-income housing programs or supportive housing.
• Bilingual in English and Spanish.
