Customer Service Representative
Location: Sacramento, CA
Key Responsibilities:
Complete eligibility screenings, intake assessments, and gather required documentation.
Develop and monitor individualized service plans; update progress regularly.
Connect participants to housing, financial, and supportive resources.
Maintain accurate documentation and enter HMIS data within required timelines.
Conduct follow-ups, reassess needs, and adjust plans to support participant progress.
Ensure compliance with confidentiality and program guidelines.
Provide guidance on budgeting, benefits, and stabilization services.
Participate in case conferences and prepare progress updates.
Required Skills & Qualifications:
Knowledge of social services, public benefits, and community resources.
Experience supporting individuals facing homelessness, mental health, or substance-use challenges.
Skills in motivational interviewing, crisis intervention, and trauma-informed care.
Bachelor's degree in social services or related experience.
Proficiency with Microsoft Office and case management systems.
Ability to work flexible hours and maintain confidentiality.
Reliable transportation and ability to pass a background check.
Preferably: Bilingual in English and Spanish.
