Case Manager (Onsite | Sunday–Thursday, 9:00 AM–6:00 PM)
We are seeking a compassionate and organized Case Manager to support individuals experiencing homelessness.
Role Overview
The Case Manager provides direct support to unsheltered and sheltered clients, helping them access housing and essential services while maintaining accurate documentation and compliance with program and funder requirements.
Key Responsibilities
Initiate contact and build working relationships with unsheltered individuals to support housing placement and service access
Engage and support clients when services are delayed or not immediately obtained
Assist clients with housing readiness documentation (ID, Social Security card, income verification, etc.)
Support completion of housing applications and accompany clients to housing-related appointments as needed
Maintain complete and accurate client records, daily activity logs, mileage logs, and required reports
Enter all client data into HMIS or other homeless management databases
Ensure compliance with program guidelines and funder requirements
Required Qualifications
High School Diploma required; Associate’s degree preferred
Minimum of 1 year of case management experience
Experience or background in substance abuse and recovery services
Strong computer skills and ability to complete required trainings
Ability to listen with sensitivity and respond to diverse needs with professionalism and empathy
Ability to demonstrate tact, courtesy, and a positive, client-centered approach
Familiarity with Harm Reduction and Housing First models of service delivery
Valid California driver’s license with no more than two points on driving record
Adequate auto insurance
Pay Details: $22.00 to $24.00 per hour
